Provides outreach and case management service on site and in the community to target population of homeless adults with mental health and/or substance use disorders. Assists in connecting PATH contacts with mental health, chemical dependency, and housing services. Assures that services are congruent with age, strengths, supports and cultural framework of the individual being served.
Provides a range of assessment and case management services in the community to assist in both recovery and entry into behavioral health services including mental health and substance use disorders treatment in conjunction with Medication Assisted Treatment (MAT) providers. Assures that services are congruent with age, strengths, supports and cultural framework of the individual being served.
The Intake Clinician conducts clinical assessments with clients seeing services at Peninsula Behavioral Health (PBH). In addition, the Intake Clinician assists the Access Coordinator with the development and implementation of policies and protocols to assure timely and appropriate client access to clinical services, and maintains appropriate connections with referral sources throughout the community.
To function as a member of a multidisciplinary team while demonstrating the clinical skills and experience to assess, plan, develop, coordinate, and provide treatment and support services to clients and families. To provide education, consultation to families, and crisis intervention services to clients and families assigned to the team. To improve the operations of Peninsula Behavioral Health (PBH) in support of its stated mission. To provide the highest level of customer service to internal and external customers
JOB PURPOSE: Provides a range of substance use disorders and co-occurring disorders treatment at Peninsula Behavioral Health, working within a multi-disciplinary team.
Provides clinical assessments, crisis intervention, case management and psychotherapy to adults in consultation with a multidisciplinary team. Manage a caseload of adult clients and provide services in accordance with funding guidelines and requirements.SIGNING BONUS: $5,000
